HARTWATER'S APPROACH TO LIQUID RHINOPLASTY
The nose is the riskiest and most technically demanding area in all of injectable aesthetics. The vascular anatomy of the nose includes vessels that connect to the ophthalmic artery, which supplies blood to the eye. This is why severe complications, including vision changes, are documented (though rare) risks of liquid rhinoplasty performed without appropriate clinical training. The nasal anatomy is also small, densely structured, and unforgiving of imprecise injection. Liquid rhinoplasty is not a treatment patients can simply pick off a menu.

COMMON LIQUID RHINOPLASTY INDICATIONS

Dorsal Hump Camouflage
Smoothing the appearance of a bump along the bridge of the nose. Filler is strategically placed above and below the hump to create a straighter profile when viewed in side angle. This is one of the most common reasons patients seek liquid rhinoplasty. An important clinical reality: filler cannot remove or reduce a dorsal hump. It camouflages the hump by changing the surrounding contour. Patients with very prominent dorsal humps may not achieve complete correction with liquid rhinoplasty and may be better served by surgical reduction. Lindsey assesses this at consultation.

Nose Tip Refinement
Defining, subtly projecting, or refining the shape of the nasal tip. Tip work is technically demanding because the tip has high vascular density and limited room for filler placement. Bridge Augmentation

Strategic bridge augmentation can meaningfully reshape the side profile of the nose without any structural intervention.

Touching up the results of a prior surgical rhinoplasty to address minor irregularities, asymmetries, or residual concerns that the patient noticed after surgical healing completed. Post-surgical correction is its own technical category because the underlying nasal anatomy has been altered by the prior surgery, which can affect vascular distribution and tissue response. Liquid rhinoplasty is not a substitute for surgical rhinoplasty. It is a fundamentally different procedure with different capabilities, different outcomes, and different patient profiles. The table below maps the differences so you can understand which option may be right for your specific case.​
Liquid rhinoplasty is the right answer for patients who want camouflage, subtle refinement, addition, or correction without committing to surgery. For patients who are uncertain about whether they want to commit to surgical changes, liquid rhinoplasty can also serve as a kind of preview of a particular aesthetic before deciding on a permanent procedure.

Patients seeking nose reduction (a smaller nose or narrower nose), patients with significant structural concerns, and patients with prominent dorsal humps or major asymmetries are typically better served by surgical rhinoplasty. VASCULAR SAFETY AND THE CLARIUS ULTRASOUND DIFFERENCE
Liquid rhinoplasty has the highest vascular complication risk profile of any common dermal filler treatment. The vessels in the nasal region connect to the ophthalmic artery, which supplies blood to the eye. When filler is accidentally injected into a vessel, blood flow can be blocked to the surrounding tissue, and in rare cases, this can result in serious complications including vision changes. Liquid rhinoplasty is one of the higher-risk treatments performed at Hartwater. We do not minimize this. Patients considering this procedure should enter the decision with an appropriate understanding of the risk profile.

Most injectors who perform liquid rhinoplasty manage vascular risk through anatomical training and injection technique. Experienced injectors understand where the facial vessels run, use specific injection patterns designed to avoid vessels, and know how to manage the rare event of a vascular occlusion. This is the current industry standard. But the risk is never zero, even with the most experienced injector, because the injection is technically blind with respect to the underlying vasculature. The vessels are not visible on the surface of the skin, and even the most skilled injector cannot see exactly where each vessel runs in a specific patient's nose.

Lindsey injects liquid rhinoplasty with the Clarius ultrasound device, which fundamentally changes this risk equation. The Clarius is a portable diagnostic ultrasound that allows Lindsey to map the nasal vessels in real time before and during injection. With ultrasound visualization, the vessels are no longer invisible. Lindsey can see exactly where each vessel runs in your specific nasal anatomy, plan injection trajectories that avoid them, and confirm filler placement in real time. In the unlikely event of a vascular occlusion, the ultrasound allows her to pinpoint the exact location and target dissolution with hyaluronidase precisely, dramatically reducing the time to restored blood flow.

Lindsey has not had a vascular occlusion occur from her own injection work in her 15+ years of practice. She has, however, personally resolved vascular occlusion complications presented by patients who were treated by other injectors seeking emergency dissolution. A liquid rhinoplasty treatment at Hartwater typically takes 30 to 45 minutes. Topical numbing is applied to the nose before injection. Lindsey then uses the Clarius ultrasound device to map the vessels in your specific nasal anatomy before any filler is placed. Filler is injected with precision, with the ultrasound used to confirm safe placement throughout. Lindsey is in the room with you for the entire treatment.

Mild swelling and slight tenderness in the nose are expected for 7 to 14 days following treatment. Bruising at the injection sites is possible. Lindsey will provide detailed post-treatment instructions before you leave, including specific guidance about glasses, masks, sleep position, makeup, and incidental contact during normal daily activity. The nose has unique post-treatment care considerations because of its exposure to all of these factors.

Most liquid rhinoplasty results are visible immediately. Final settled results typically appear at 2 to 3 weeks as initial swelling resolves and the filler integrates with the surrounding tissue.
